    It's never too late to start exercising. 

    Care Ventures Cooperative aims to provide programming that focuses on the physical aspect of wellness for the elderly population. We hope to improve the quality of life for our residents through group exercise classes, cardiovascular programs and individualized plans of care.  Improvements are forseen not only in physical aspects such as balance, strength and flexibility, but also better mood, socialization and ADL functioning within the senior population.

    Physical activity is also one important intervention continuing forward for fall prevention programming.  New physiologic assessments, in-depth balance assessments, and improved workouts are some of the efforts that are geared more towards preventing falls.

    Year 2 Goal (by September 30, 2011)

    - Maintain or improve the average fall rate from Program Year 1 (10-2009 thru 9-2010)

    Fall Rate = # of residents who fell during program year divided by # of residents at risk of falling (total # of SNF population during program year using monthly censu averages)

    Year 3 Goal (by September 30, 2012

    -Maintain or improve the average fall rate from Program Year 1 (10-2009 thru 9-2010)

    Year 2 Goals (by September 30, 2011)

    (1)- Increase to 5% the proportion of community members identified with fall risks that engage in additional episodes of exercise or balance programming 8 times a month or more (Baseline = 0%)

    (2)- Increase to 2% the proportion of community members with history of falls who report fewer falls upon reassessment (Baseline = 0%)

    Year 3 Goals (by September 30, 2012)

    (1)- Increase to 7% the proportion of community members identified with fall risks that engage in additional episodes of exercise or balance programming 8 times a month or more

    (2)- Increase to 3% the proportion of community members with history of falls who report fewer falls upon reassessment
